First Visit
Please come to your first visit with a Patient Form completed. Click the buttons below to download forms. In addition, please bring any x-rays that your referring dentist may have given to you.
Office HoursCall to schedule an appointment with our office. We have varied hours to accommodate many schedules. Our office hours are:
Monday 12 pm – 8 pm
Tuesday 9 am – 4 pm
Wednesday 9 am – 5 pm
Thursday 12 pm – 7 pm
Friday 9 am – 2 pm
Dental InsuranceOur office participates with many insurance providers, from major carriers to local union plans. For a comprehensive list, please call us. As a courtesy, we will file your claim with your dental carrier. For more detailed information on dental insurance, please feel free to discuss questions with our staff.
Payment OptionsYour dental health is very important to you and to us. We are here to help you with your investment to a Healthy Smile. We accept cash, check, Visa and Mastercard as payment options.
